Position Description:

Unrestricted license in California Conduct rounds with physicians to meet with patients to discuss placement orders Assist patients with a specific plan for medication management Include patient in discharge planning Assist patients with the identification of a support network, instructions for future care and follow-up Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors Come learn correctional healthcare as you use our cutting-edge resources, including our award winning electronic operating system at our sites in San Diego County.
