Responsibilities Develop, implement, and review personalized care plans tailored to each resident's medical conditions and preferences. Supervise and mentor nursing staff, including registered nurses (RNs) and licensed practical nurses (LPNs), ensuring high-quality patient care and adherence to best practices. Oversee documentation review, medical records management, and ensure compliance with HIPAA regulations to protect patient confidentiality. Coordinate with interdisciplinary teams to facilitate smooth communication regarding residents' progress and care requirements. Manage shift schedules, monitor staffing levels, and ensure proper coverage for all home health visits. Conduct regular assessments of residents' conditions, including physiology and anatomy knowledge, to adjust care plans accordingly. Ensure accurate documentation of all care activities using EMR (Electronic Medical Records) systems and EHR (Electronic Health Records) systems while maintaining compliance with Medicare standards. Skills Extensive experience in assisted living facilities, nursing homes, or hospice care environments. Strong background in home health nursing with proven supervising and management experience. Knowledge of MDS (Minimum Data Set) assessments and their role in resident care planning. Familiarity with acute care procedures and medical terminology essential for effective communication and documentation review. Leadership skills with the ability to manage shift operations efficiently while fostering a team-oriented atmosphere. Proficiency in EMR/EHR systems for accurate recordkeeping and documentation management. Understanding of HIPAA regulations, Medicare guidelines, and nursing administration best practices. Excellent communication skills to collaborate effectively with residents' families, healthcare providers, and team members.
