This position will provide coordination of multidisciplinary services for identified children and families, as well as collaboration with community partners and referral sources. The Skills Coach will provide case management services and skills training / development services to children and families in the office, home, and in the community.

WHAT IS THIS POSITION RESPONSIBLE FOR?
Coordinate services for children and families referred to Development Clinic. Coordination of care includes but is not limited to communicating with all parties involved in the child's / family's treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process. Assist with low barrier access to services for families. Provide community-based skills training and development services for children and families based on needs. Maintain appropriate records of services to reflect patient progress; develop treatment plans and goals in conjunction with families and referral sources. Maintain documents, collect data, and submit reports according to established time frames. Provide screening for appropriate services; communicate professionally, make referrals, and coordinate services with community agencies. Attend and participate in team meetings with families and multidisciplinary team of providers. Advocate for families and children with complex needs. Participate in ongoing staff training and development activities as assigned to enhance job performance. Provide emergency counseling and crisis intervention when necessary.
WHAT'S REQUIRED FOR THIS POSITION?
Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required. Experience working with children who have a Dual Diagnoses of both Serious Emotional Disturbance and Neurodevelopmental Disorder is preferable Possess the ability to assess emergency situations and react in an appropriate and therapeutic manner. Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance. Candidates must pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check, and drug screen.
