Overview If you have experience working directly with individuals in behavioral health or substance use treatment and you're ready to take on more responsibility, the Intake, Assessment & Referral Specialist role may be your next move. This isn't an entry-level position—but you don't need to have spent years working as a clinician, either. At Cornerstone Healing Center, our Intake, Assessment & Referral Specialists play a critical role in transitioning individuals from the admissions process into appropriate treatment. When a prospective client arrives for intake, you'll help create a welcoming first in-person experience while completing the screenings, assessments, documentation, and care coordination necessary to determine appropriate placement and level of care. Responsibilities:
Conduct initial in-person screenings and clinical assessments to help determine eligibility, acuity, level of care, and placement needs. Complete ASAM biopsychosocial and/or LOCUS assessments and other required behavioral health screenings. Gather and document client history, presenting concerns, risk factors, and relevant clinical information. Identify potential safety concerns and follow established protocols when a client may be at risk of harm to themselves or others. Assist with preliminary diagnosis and appropriate placement recommendations within the scope of your qualifications. Coordinate internal placement and available bed space based on clinical criteria and program capacity. Coordinate transfers, step-downs, and external placements when clinically appropriate. Work closely with Admissions, Clinical, Medical, and Navigation teams to ensure a smooth transition into treatment. Support virtual screenings and assessments when needed. Communicate with families, referral sources, external facilities, and community partners as appropriate. Complete timely, accurate intake, assessment, referral, and placement documentation. Maintain referral and admissions activity within the CRM.
Qualifications:
Must be 21 yers or older and have or ability to gain; CPR/First Aid Certification Nasal Naloxone Opioid Reversal Training Two-Step Test Freedom from TB Behavioral Health Technician (BHT) Qualifications must meet or exceed the following criteria: Education and Duration of Education; A high school diploma (2+ years) or A high school equivalency diploma (3+ Months) or Associate's degree (6+ Months), or A College Degree (3+ years, or more), or A Master's Degree (1+ years or more). Duration of Experience must be 6-Months full time in the field or higher. Type of Experience; Behavioral Health Services, Mental Health or Substance Abuse; or Sober Living Management / Group Homes Experience; or Hospital / Medical facility / Psychiatric Care; or DDD Services /
Special Education Knowledge & Abilities Required:
Have experience working in behavioral health, mental health, substance use treatment, psychiatric care, sober living, group homes, or a related human-services environment. Understands substance use disorders, behavioral health conditions, and co-occurring mental health needs. Is comfortable interacting with individuals who may be in crisis, early recovery, or experiencing significant life challenges. Can communicate professionally and compassionately while maintaining appropriate boundaries. Is organized, dependable, and able to manage multiple priorities in a fast-moving environment. Can exercise sound judgment while recognizing when clinical consultation or supervision is needed.
