Job Description

Job Summary We are seeking a compassionate, mission‑driven Community Education Coordinator to lead hospice‑focused outreach and education throughout the communities we serve. This paid position plays a vital role in increasing awareness of hospice services, promoting timely access to end‑of‑life care, and strengthening relationships with healthcare providers, senior communities, faith‑based organizations, and underserved populations. The ideal candidate is a motivated professional with experience in healthcare outreach, social work, public health, or community engagement. This role is essential in helping individuals and families understand hospice philosophy, available benefits, and the supportive services that improve quality of life during serious illness. Responsibilities Increase understanding of hospice care and eligibility Support families and caregivers with resources and guidance Build partnerships with physicians, hospitals, SNFs, ALFs, and community groups Promote early and appropriate hospice referrals Enhance community trust and visibility of our organization Develop, coordinate, and execute community-based educational programs focused on health promotion, disease prevention, and wellness strategies. Collaborate with local organizations, healthcare providers, and community leaders to identify needs and tailor programming accordingly. Conduct outreach activities to engage underserved populations and ensure accessibility of services. Adheres to all laws, statutes and regulations regarding anti-kickback, lawful marketing activities (gifts and services), patient solicitation, and accurate marketing materials. Requirements Bachelor's degree in marketing, Communications, or related field preferred. At least two years' experience in hospice marketing preferred. Ability to market successfully and deal tactfully with customers and the community. Ability to build trust and long-term relationships. Ability to work independently and as part of a team. Demonstrates good communications skills, negotiation skills, and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Must possess a valid state driver's license and automobile liability insurance and the ability to travel within site regions.
