PER APPLICATION. IF YOU ARE INTERESTED IN PROVIDING SERVICES IN MULTIPLE CATEGORIES, YOU MUST SUBMIT SEPRATE APPLICATIONS FOR EACH.

You are welcome to provide any supporting documentation (resume, outline of the proposed services, licenses, certifications, etc.) that you feel may be beneficial in evaluating your proposal. You will be able to attach supporting documentation to your on-line application. Must successfully pass background check and drug screening prior to commencement of any services. Background check fee is $68.25
