POSITION CONCEPT

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Facilitate team in training, supporting and supervising individuals with disabilities in groups and individually in various skill areas focusing on advancing the individual's path to integrated employment and advancing the individual's path to community connections/membership in accordance with Individual Service Plans (ISP).
• Lead and provide assessments and input for ISPs; participates in staffing meetings and development of IPs for individuals; assists in establishing outcomes and setting up programs to accomplish objectives; prepares plans for instructional activity; obtains necessary materials and equipment for training.
• Must be comfortable assisting with personal care as needed
• Must be able to drive transportation vans

EDUCATION/TRAINING/EXPERIENCE

• High School Diploma/GED Required
• Driver License with no more than 5 points SKILLS and

ABILITIES

• Develops meaningful relationships with clients and reports praises and/or concerns
• Patient and calm communication skills to communicate with team and individuals with disabilities.
• Can think outside of the box and love to learn/share new ideas and work to see them practice
• The ability to deal with sensitive people in time of need.
• Can pass a drug screen, background check, and has a valid drivers license with less than 6 points on driving record (for driving individuals around the community).

Pay:

$18.00 - $20.00 per hour
