• The purpose of the admissions/marketing coordinator is to assist new residents with the transition into the skilled nursing facility.

This includes working with hospital staff on the details of transferring from the hospital to the SNF, working with patients and families to complete admission paperwork, verifying patients are appropriate for SNF placement and serving as a member of the facility department head team. This role is also responsible for completing a monthly marketing plan and for marketing the facility in the community. Hang On...

Sales & Community Relationship Building:

Routinely visit hospitals to build relationships with case managers and discharge planners Represent our facility as the go-to choice when hospitals are placing patients Meet face-to-face with prospective residents and their families to sell the value of our care Conduct facility tours that turn "just looking" into "let's do this" Build and maintain a referral pipeline across hospitals, physicians, and community partners Serve as the face of our facility

• inside the walls and out in the community Admissions Support (Once You've Closed the Deal): Coordinate incoming patient admissions with the nursing team Greet newly admitted patients and escort them to their rooms Review and help complete admission paperwork Keep accurate records of bed availability and current patients Stay current on Medicaid/Medicare and state/federal admissions requirements
