The IWC case Manager will provide assessment of needs, service planning, reassessments, linkages and referrals to public and private services and programs, information dissemination, advocacy, mental health assessment, and service plan follow-up planning. This position will also support and coordinate monthly educational workshops and outreach events.

Associate Director of Health Center Operations Primary Functions:
Provide comprehensive and culturally sensitive case management services to all patients and clients, including intake assessment, benefit assessment, financial goal setting, educational goal setting, money management, and financial literacy education. Act as a point of contact for the local community, including but not limited to hospitals, health care providers, emergency responders, and social service providers. Maintain current knowledge of state and federal programs for community members. Follows procedures during a crisis intervention of medical and/or psychiatric emergencies on and off-site. Monitor behavioral functions and report as needed to ensure a safe, welcoming environment for patients and clients. Assist in the referral and linkage to community resources including, but, not limited to behavioral health or social services in the community. Assist in the scheduling and confirmation of patient and client appointments. Collaborate with internal and external resources to ensure that appropriate referrals are made and follow-up services are provided as needed. Connect families with needed and available community resources. Follow up with referral agencies to ensure the use/success of the referral. Document all contact with or on behalf of a participant in the participant's designated chart and as required by the funding source. Weekly case plan development, progress monitoring, advocacy, and referrals. Provide translation services and support for IWC program participants. Participate in staff meetings, staff training, and other duties as assigned. Coordinate transportation for program participants if needed to locations within the greater Illinois area. Provide practical help and mentoring, advocacy, coordination, side-by-side individualized support, problem solving, and direct assistance to help patients and clients obtain the necessities of daily living. Promote the patients' and clients' self-advocacy skills. Develop and maintain required IWC program records, reports, and statistical data. Familiarizes and complies with all VNA policies and procedures. Meets standards of behavior expectations. Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment. Other duties as assigned.
Compensation:
Hourly Range:
$21.70 - $25.00, depending on experience.
Incentives:
Earn up to $500.00 quarterly based upon meeting productivity goals.

Qualifications:
Bachelor's degree in the social work or related social services field required. A licensed professional is preferred. Must be able to write, speak, facilitate, and translate in both English and Spanish. Excellent communication and interpersonal skills (verbal, and written) Case Management and Outreach experience required. Electronic Medical Records experience preferred. Experience working in a community-based/social service setting. Prior knowledge and experience working in a diverse community. Culturally competent and understanding the immigrant and refugee community in the surrounding area. Highly organized and detail oriented. Ability to juggle multiple tasks, set priorities, and follow through on daily tasks in a timely fashion. Ability to handle confidential information professionally and appropriately. This position requires travel. Valid Driver's License, Car Insurance, and Car is essential.
