A Preschool or Childcare Director manages and directs programs for very young children. Supervises child care workers and pre-school teachers. Proposes policies, program fees and budgets for approval by a program board or administrative agency or implements policies of a national organization. Meets with parents and staff to discuss the development, learning and behavior of children in the program or center. Orders supplies and monitors maintenance and cleaning at a facility. Ensures that the program or center meets state and federal guidelines.

We're in search of an experienced social worker for our skilled rehab, assisted living, and long-term care residents at the Kroontje Health Care Center. The position will be responsible for providing social work services, active discharge planning for the short-term rehab residents, completion of assessments required for the MDS, and referrals for ancillary services. Candidates should possess a sensitivity to ongoing cultural diversities and the needs of the aging population and be trained in recognizing and assessing the emotional and social needs of residents. The ideal candidate has a history of successful positions involving direct customer service, social services, coordination, and problem-solving, and teamwork. Strong communication and collaboration skills are a must. Key Responsibilities Complete comprehensive psychosocial assessments for new admissions and ongoing resident evaluations. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Provide emotional support, counseling, and crisis intervention to residents and their families. Assist residents and families in adjusting to assisted living, short-term or long-term care placement and changes in health status. Coordinate admissions, transfers, and discharges while ensuring continuity of care. Facilitate family meetings and participate in resident care conferences. Connect residents and families with community resources, financial assistance programs, and support services. Advocate for resident rights, dignity, autonomy, and quality of life. Assist with advance care planning and provide support during end-of-life care. Maintain accurate, timely, and confidential documentation in accordance with facility policies and regulatory requirements. Ensure compliance with all federal, state, and local regulations governing long-term care. Participate in quality improvement initiatives and interdisciplinary team meetings.
Qualifications Required:
Knowledge of psychosocial assessment, care planning, and resident advocacy. Excellent communication, organizational, and interpersonal skills. Ability to work collaboratively with residents, families, physicians, nursing staff, and other healthcare professionals.
Preferred:
4-year degree in Social Work, Psychology, Sociology, Education, Counseling, or other human services related degree. Previous experience in assisted living, long-term care, skilled nursing, rehabilitation, or geriatric services. Experience with Medicare, Medicaid, discharge planning, and regulatory compliance.
