The OSO Intake Specialist is responsible for managing and processing referrals received through Internal Referral Portals (IRP) and External Referral Portals (ERP) as well as community referrals for Home Health and Hospice services. This role ensures obtaining accurate intake documentation, timely communication with referral sources, and efficient data entry into the EMR system to support smooth patient onboarding and care coordination to the local region intake teams. Monitor and process incoming referrals via IRP and ERP, Forcura Fax systems and community referral platforms. Review referral documentation for completeness, accuracy, and compliance. Enter referral data into the Provider Link andHCHB EMR
system tools. Effectively communicate across multiple referral platforms and correspondence streams (email, faxing etc.). Effectively communicate with Business Development, Navigators and TCCs/eTCCs teams and community referral sources. Establish effective processes to quickly obtain timely referrals, obtain missing documentation or collaborate to obtain needed referral information. Coordinate with OSO clinical intake specialists to complete both financial and clinical clearance to ensure timely assignment and scheduling at the region level. Verify payer source and in-network insurance acceptance to confirm 1st level financial clearance and referral process success. Maintain accurate logs and tracking systems for referral status and follow-up. Ensure compliance with HIPAA and internal policies regarding patient information. Escalate urgent or complex referrals to appropriate clinical and operational leadership teams on both the OSO and region levels. Collaborate with internal departments (Authorization, Billing, scheduling) to support smooth transitions of care. Strengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high-quality service provider, while collecting and reporting referral data and key customer trends. Perform other related duties as requiredQualifications Education:
Requires high school diploma. Bachelor's Degree Preferred in Healthcare Administration or related field.Experience:
1 Year of Administrative Healthcare Experience. 1-3 years of experience preferably in Home Care preferred. Knowledge, Skills and Ability Requirements Familiarity with IRP and ERP and community referral systems and workflows. Knowledge of HIPAA and patient privacy regulations. Strong data entry and documentation skills. Excellent verbal and written communication. Organizational and time management skills. Ability to work in a fast-paced, detail-oriented environment.
