Job Description

ESE TRANSITION SPECIALIST JOB DESCRIPTION
: The ESE Transition Specialist reports to the Transition Coordinator, and performs a variety of duties related to supporting students' progress in pursuit of attaining Individual Education Plan (IEP) Goals.
REQUIREMENTS
: Minimum of a Bachelor's degree and eligible for ESE certification.
DUTIES :
Supports the Transition Coordinator and other transition staff members as follows: Works under the direction of the Transition Coordinator Assists students in writing and implementing IEP Goals Assists students in the classroom setting Documents case notes, job logs, and quarterly progress tracking reports. Coordinates data collection and conducts IEP Meetings Participates in community agency meetings and workshops Works collaboratively with District High School ESE Contacts Provides job development skills training and assistance securing employment Provide follow-along services for students at their job sites Presents Transition Program to graduating high school seniors who elect to defer their standard diploma
