Position Summary:
Ready to break out of the clinic or hospital setting and step into a role where you can practice with purpose? As a Wound Care focused Nurse Practitioner (NP) , you will provide on-site rounding and advanced wound care across senior living communities. You will manage complex cases, build long-term patient relationships, and operate with a high level of independence. Responsibilities:
Provide on-site primary care and wound care management in assisted living, memory care, and SNF settings. Manage complex wounds including pressure injuries, ulcers, and post-surgical cases. Perform chronic disease management and preventative care. Reduce hospitalizations and ER utilization through proactive care. Conduct assessments, diagnostics, medication management, and follow-up. Collaborate with physicians, PAs, nursing staff, therapists, and care teams. Maintain accurate EMR documentation and coordinate care across settings.
Qualifications:
Active Minnesota Nurse Practitioner (NP) license in good standing. Experience in wound care, geriatrics, primary care, or post-acute care preferred . Familiarity with SNF rounding , post-acute care , or mobile primary care models . Strong clinical judgment and ability to manage complex patient populations. Valid driver's license and reliable transportation. Military Occupational Classification (MOC)
