Position Overview:

Kings View is seeking a Care Manager (Peer or Paraprofessional) to support Managed Care services. This role coordinates care, supports members, and promotes whole-person wellness.

Key Responsibilities:

Outreach, engagement, assessments, and care planning Coordinate ECM and support services Connect members to housing, transportation, and benefits Advocate with providers and agencies Coordinate discharge planning and referrals Maintain timely documentation Collaborate with multidisciplinary teams

What You'll Bring:

High School Diploma or equivalent One year relevant experience Valid California Driver License Experience with homeless, SMI, SUD, or justice-involved populations BA/BS preferred; bilingual preferred
