Position Summary The Care Manager is responsible for engaging members and their support systems while coordinating whole-person care services. This position works collaboratively with multidisciplinary teams to address behavioral health, physical health, housing, transportation, and other social determinants of health to support wellness and recovery. Key Responsibilities Conduct outreach, engagement, assessment, and care coordination services Develop and implement individualized care plans Connect members to community resources, healthcare, transportation, and support services Advocate on behalf of members with healthcare and social service providers Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches Coordinate discharge planning and follow-up services Monitor treatment adherence and support self-management goals Required Qualifications High School Diploma or equivalent One year of experience working with homelessness, behavioral health, substance use, or justice-involved populations Valid California Driver's License Knowledge of behavioral health, homelessness, and community resources Strong communication and relationship-building skills Preferred Qualifications Bachelor's Degree preferred Bilingual English/Spanish preferred CNA or Medical Assistant experience desirable Why Join Kings View?
