Job Overview Under clinical and administrative supervision, the CORE Medicaid Program Paraprofessional provides community-based, recovery-oriented support to adults, and families receiving behavioral health services. The position helps individuals carry out treatment-plan objectives, strengthen daily living and self-management skills, connect with natural and community supports, and maintain stability in the least restrictive setting. The Paraprofessional works within the limits of assigned qualifications and agency policy and does not independently diagnose, develop treatment plans, or provide services that require a professional license. Responsibilities Duties include, but are not limited to, the following: Provide skill-building, resource coordination, service linkage, monitoring, and other assigned CORE supports consistent with the individual's authorized services and treatment plan. Engage individuals and families using person-centered, trauma-informed, culturally responsive, and recovery-oriented practices. Teach and reinforce practical skills related to coping, symptom self-management, communication, problem-solving, community integration, medication routines, wellness, and independent living, as authorized. Coordinate and link individuals to medical, psychiatric, substance-use, educational, vocational, housing, benefits, transportation, and other community resources. Follow agency crisis-prevention and emergency procedures, immediately notify the appropriate supervisor of safety concerns, and support continuity of care after a crisis. Complete accurate, timely, and service-specific documentation that supports medical necessity, the intervention delivered, the individual's response, progress toward goals, and the plan for follow-up. Maintain confidentiality and comply with HIPAA, Medicaid requirements, DBHDD standards, payer rules, agency policies, and applicable accreditation requirements. Participate in orientation, competency validation, field observation, and the agency's 30/60/90-day onboarding and supervision process. Attending required individual supervision, team meetings, case conferences, utilization reviews, and organizational meetings. Seek supervisory guidance when a case exceeds the Paraprofessional's training, assigned role, or scope of practice. Manage assigned caseload, scheduling, service authorizations, and productivity expectations while prioritizing quality, safety, and compliance. Complete all required training and maintain current credentials, background clearances, and other employment requirements. Perform other related duties consistent with the position, applicable regulations, and the individual's plan of care.
Education:
High School Diploma required. Bachelor's degree in social work, psychology, counseling, human services, or a related field preferred. Current CPR/First Aid, and Crisis De-escalation (CPI) certifications are required. A combination of education, training, and directly related experience may be considered only when permitted by the current service definition, payer contract, and agency policy.
Experience and Skills:
Prior experience as a paraprofessional with a CORE Medicaid agency is preferred. Experience supporting individuals with mental health, substance-use, co-occurring, or related community-service needs. Core Working knowledge of person-centered planning, community resources, professional boundaries, confidentiality, crisis response, and basic case-management practices. Ability to communicate clearly, document services accurately, use electronic health-record systems, manage time, and work independently in community settings with appropriate supervision. Ability to establish effective professional relationships with individuals served families, providers, community partners, and agency staff. Reliable transportation, a valid driver's license, and insurability under the agency's policy when travel is an essential job function. Ability to satisfy all screening, training, competency, and credentialing requirements established by the employer and applicable oversight entities.

$20.00 - $25.00 per hour
