Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. Job Summary The Program Assistant will provide support for the Assertive Community Treatment (ACT) program by assisting the program manager with scheduling, data entry, client record-keeping, in-office client communication/de-escalation, and other duties to be assigned. Job Responsibilities Complete data entry for ACT Team and sort and record all client mail. Prepare and file records for ACT Team. Prepare template for daily staff assignment schedule tactual staff assignments completed by Team Lead before morning meeting) Order necessary supplies for ACT program including miscellaneous support duties. Update and keep an inventory of all forms used by ACT program. Maintain and report daily census of ACT Team clients. Prepare monthly billings on spreadsheet/database and maintain all informational spreadsheets/databases regularly. Collect necessary data and prepare reports as needed. Prepare letters to request medical records and client authorizations and maintain log of requests and records received. Design and maintain forms. Assist with requirements of documenting interventions into MyAvatar and CaseWorthy within 24 hours of intervention. Maintain client files and complete filing regularly. Operate and coordinate the computer management information systems for the program. Answer telephone and attend to client and coworker issues in a timely and courteous manner. Manage scheduling of client appointments with team psychiatrist and RN. Deescalate clients as needed and communicate situation to team. Respect client confidentiality. Responsible for ministry policy regarding work hours, building security and all written personnel standards including working a consistent schedule per the core working hours of service line. Ensure the care model by interacting with all other service line programs. Report to and work with assigned Program Manager. Work and assist all service line coworkers when and where needed. Participate in all appropriate service line meetings and attend appropriate seminars. Other duties as assigned. Job Requirements High school diploma or GED required. Associate's degree preferred. Drug test to be completed within 30 days of hire. Understanding of mental illness and substance use. Proficiency in data entry and typing using Windows, Microsoft Word, Excel and Outlook. Excellent organizational skills, both verbal and written. Proficiency in written business communications. Attention to detail. Reliable and honest. Respect for clients and coworkers and a positive support for ACT program.
