Weekdays, evenings, rotating weekends, and holidays as scheduled Position Overview The Community Living Support Specialist (Paraprofessional) provides individualized, person-centered support to adults living in supportive living homes and community settings. The specialist helps clients develop, maintain, and improve the skills needed to live as independently as possible, participate in their communities, and achieve the goals identified in their person-centered plans. This is a community-based direct-support position and is not a 24-hour residential supervision role. Services are provided according to each client's authorized service plan, assessed needs, scheduled service hours, and personal choices. Work Environment Services may be provided in a client's supportive living home and throughout the Greensboro community. Staff must be comfortable working independently, transporting or accompanying clients in the community, and adjusting activities based on each client's needs, goals, abilities, and preferences. The position requires reliability, patience, sound judgment, professional boundaries, and the ability to remain calm when responding to behavioral, medical, or environmental concerns. Staff must arrive on time, complete scheduled services, communicate promptly about emergencies or scheduling issues, and follow all agency policies and Medicaid service requirements. Key Responsibilities Provide individualized Community Living and Support Services in accordance with the client's person-centered plan, service authorization, goals, and assessed needs. Teach, model, prompt, and reinforce independent-living skills rather than routinely completing tasks for the client. Assist clients with developing daily routines related to personal hygiene, grooming, clothing selection, meal preparation, housekeeping, laundry, organization, and maintaining a safe living environment. Support clients with budgeting, shopping, scheduling appointments, using community resources, and understanding household responsibilities. Help clients strengthen communication, decision-making, self-advocacy, problem-solving, social, and interpersonal skills. Support clients with identifying educational, volunteer, recreational, and employment-related opportunities. Accompany and support clients during approved community activities, errands, appointments, and skill-building outings. Encourage healthy choices, community inclusion, natural supports, and increased independence. Provide behavioral support using approved positive-support, redirection, and de-escalation strategies. Recognize and promptly report changes in a client's physical health, mental health, behavior, safety, or functioning. Follow applicable crisis plans, safety plans, emergency procedures, and incident-reporting requirements. Provide medication reminders only when permitted by the client's service plan and agency policy. Staff may not administer medications unless specifically authorized, trained, and assigned to do so under applicable requirements. Protect client rights, privacy, dignity, confidentiality, personal property, and freedom of choice. Maintain professional boundaries and avoid controlling a client's activities, visitors, food, money, personal property, or daily choices except as specifically authorized in the client's plan for health or safety. Complete accurate, individualized service notes describing the support provided, the client's participation, progress toward goals, and response to interventions. Accurately record service start and end times and complete electronic visit verification when required. Ensure documentation supports the services such as personal-care, or behavioral-health service. Communicate professionally with clients, supervisors, guardians, care managers, service providers, and other authorized members of the client's support team. Participate in staff meetings, supervision, training, and person-centered planning activities as required. Follow all MC2 Professional Care Services policies, confidentiality requirements, client-rights protections, and applicable state and Medicaid standards. Perform other duties within the scope of the position and the client's authorized service plan. Minimum Qualifications High school diploma or GED required. Must be at least 21 years old. Experience supporting adults with mental-health, developmental, behavioral, or independent-living needs is preferred. Coursework or a degree in human services, social work, psychology, education, healthcare, or a related field is preferred but not required. Ability to teach and encourage independence without creating unnecessary dependence. Strong communication, observation, documentation, and interpersonal skills. Ability to remain calm, respectful, and professional during stressful situations. Ability to understand and follow person-centered plans, safety plans, agency procedures, and supervisory instructions. Basic ability to use smartphones, electronic documentation systems, email, and electronic visit-verification applications. Valid driver's license and an acceptable driving record. Reliable transportation and automobile insurance when transporting clients or using a personal vehicle for work. Must successfully complete required background checks, healthcare-registry checks, drug screening, and other pre-employment requirements. CPR and First Aid certification or willingness to obtain certification before independently providing services. Ability to complete all required orientation and ongoing training. Flexible availability, including some evenings, weekends, and holidays. Physical Requirements Ability to stand, walk, bend, reach, and remain active for extended periods. Ability to safely assist clients with approved daily-living activities within the employee's training and assigned responsibilities. Ability to accompany clients in home and community environments. Ability to respond promptly and appropriately during emergencies. Ability to lift or carry up to 25 pounds when necessary for routine job responsibilities. Available Positions Weekday and Evening Weekend Rotating Schedule PRN/As-Needed Schedules are based on client authorizations, service needs, and agency staffing requirements.
