Key Accountabilities:
Leads and manages a team of staff to ensure effective coordination of patient care across disciplines. Directs the planning, implementation, and evaluation of quality patient care for SHH patients. Fosters consistent communication with department leaders and direct reports to support collaboration and alignment of priorities. Builds visibility and accountability across the organization, strengthening relationships with internal and external stakeholders. Represents the department on committees and in special projects, contributing to organizational strategy and decision-making. Provides oversight and guidance to clinical teams, ensuring access to appropriate resources and support in alignment with scope of practice requirements. Reviews and interprets patient/family health data (physical, psychological, and social factors) to inform clinical team decision-making. Aligns customer needs with appropriate internal resources, including skilled care, personal care, alert follow-up, and other healthcare services. Ensures compliance with federal, state, and local regulations; monitors and implements changes as required. Establishes and maintains efficient systems and communication channels to support continuity and quality of care. Evaluates agency operations and systems, identifying opportunities to improve workflow, productivity, and resource allocation. Performs other duties/projects as assigned. What is Required? Graduate from an accredited RN program. 2+ years experience within a home health setting is required, field experience preferred Prior management experience is required. Proficiency with MS Office Suite (Word, Excel, Outlook, etc.) Demonstrated knowledge of the healthcare industry and telehealth. Experience in surgical nursing, critical care, and experience with Homecare Homebase EMR are preferred. Must have current drivers license, reliable transportation and insurance in accordance with the laws of the state. Must be able to sit, stand, bend, lift and move intermittently and be able to lift at least 25lbs
