  • Full-time Overview As a Discharge Planner, you will be responsible for ensuring that appropriate discharge plans are in place prior to hospital discharge and appropriate clinical services are coordinated for the member, to support the successful transition in the community setting and reduce the risk of readmissions.
In addition you will be responsible for participating in inpatient calls, discharge ready calls with the RBHA, staffing, and providing support to clinical team in identifying and developing clinically appropriate discharge plans. Benefits Multiple medical plans

Qualifications Requirements:
A High School Diploma/GED and four years behavioral health experience; OR An Associate's degree and two years behavioral health work experience; OR Bachelor's degree in non-behavioral health related field one year behavioral health work experience; OR Bachelor's degree in a behavioral health related field One year of related work experience in an inpatient setting or utilization management setting, preferred. Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR) Must be able to obtain a Fingerprint Clearance Card Must be 21 years of age Background Check Minimum 1 year recovery from drugs and/or alcohol Minimum typing speed of 25 wpm
