The Healthcare Surveyor / Intake Specialist will: Triage reports of abuse, neglect, exploitation, suspicious injuries, environmentally hazardous conditions, and death for individuals on the developmental disability waiver, Mi Via Self-Directed waiver, the Medically fragile waiver, and the State General Fund; Be responsible for taking abuse, neglect, exploitation (ANE) reports through the Incident Management Bureau (IMB) hot line, monitoring the ANE on-line reporting system, and procuring an Immediate Action and Safety Plan/Safety Verification; Obtain information to determine jurisdiction and authority to assign pursuant to state regulations and make screening decision; Conduct enhanced screenings for reports of ANE and will be responsible for follow up interviews and documentation to assist in screening decisions. How does it get done? The Healthcare Surveyor / Intake Specialist is responsible for: Receiving calls through the ANE hot-line and receive and triage online ANE reports via on-line reporting portal; Conducting enhanced screenings to make accurate screening decisions of the reports filed with IMB; Creating a tip sheet containing the information reported through the hot-line or on-line portal, and obtain an Immediate Action and Safety Plan/Safety Verification; Making a screening decision; On-call working hours. The Individuals who received Medicaid waiver services such as the DD waiver, Mi Via waiver, the Medically Fragile waiver, and the State General Fund; Others affected include guardians, case managers, provider agencies, family, natural support, and the general public. Minimum Qualification Associates Degree from an accredited college or university in Nursing, Architecture, Social Work, Sociology, Psychology, Rehabilitation, and/or Criminal Justice and two (2) years directly related experience in community based healthcare services and case management. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience. Employment Requirements Must possess and maintain a valid Driver's License. Must obtain a Defensive Driving Certificate. Working Conditions Work is performed in an office setting and in the field at community program sites. Out-of-state travel for training, and extensive statewide, with the occasional overnight travel is required. On-call duties may be required.
