Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar. Perform initial and ongoing assessments using approved tools and procedures. Evaluate functional status, service needs, supports, and risks. Complete all required documentation accurately and on time. Communicate assessment findings and follow-up needs to designated parties. Coordinate with members, caregivers, case managers, MCOs, and agency staff as needed. Maintain compliance with policies, confidentiality rules, and reporting expectations. Participate in training, quality assurance, and calibration activities. Minimum Qualifications A Bachelor's Degree or a minimum of 4 years of professional experience in the field of intellectual, developmental, and/or physical disabilities required. Effective organizational, interpersonal, communication and leadership skills. Effective computer skills with familiarity with suite of Microsoft Office programs.
