EXPERIENCED LICENSED PHYSICAL THERAPIST

Family Connect Care is a compassionate memory care community dedicated to enhancing the quality of life for seniors living with Alzheimer's disease and other forms of dementia. Our team is committed to providing person-centered care in a safe, engaging, and supportive environment. We are seeking a caring and experienced Part-Time Physical Therapist to join our interdisciplinary team and help residents maintain mobility, independence, safety, and overall well-being. Position Summary The Physical Therapist will evaluate residents' physical functioning and develop individualized treatment plans that promote strength, balance, mobility, fall prevention, and functional independence. This role requires a compassionate professional who understands the unique needs of older adults with cognitive impairment. Responsibilities Conduct physical therapy evaluations and reassessments for residents Develop and implement individualized treatment plans Provide therapeutic exercises, gait training, balance training, and mobility interventions Collaborate with nursing staff, caregivers, physicians, and families regarding resident care plans Educate staff on safe transfer techniques, mobility assistance, and fall prevention strategies Maintain accurate documentation in accordance with state and federal regulations Monitor resident progress and modify treatment plans as needed Support residents with dignity, patience, and empathy Qualifications Licensed Physical Therapist in the state of California Graduate of an accredited Physical Therapy program Experience working with seniors, geriatrics, or memory care populations preferred Strong communication and interpersonal skills Ability to work collaboratively within a multidisciplinary team Compassionate, patient-centered approach to care CPR certification preferred Schedule Part-time position Flexible scheduling available Benefits Competitive hourly compensation Flexible work schedule Supportive team environment Opportunity to make a meaningful difference in residents' lives How to Apply Please submit your resume and cover letter to: busoffice@familyconnectcare.com Family Connect Care is an Equal Opportunity Employer committed to creating an inclusive and welcoming environment for all employees and residents.
