Family Partner - Youth and Family Services 2304

General Description Implements family support services for emotionally disturbed children and adolescents. Provides support, information, referral, follow-up, and technical assistance to families and general consultation to staff on children and youth. Develops and maintains data collection system indicating the number of referrals, resources/services, unmet needs of families, and barriers to obtaining services for all interested parties. If Family Partner is a QMHP-CS, the following duties are also required.

• Assist in accessing emotional, behavioral, social, educational, medical, and other appropriate services using the Uniform Assessment tool.
• Assesses client's needs and authorizes services to meet the identified needs.
• Coordinates the development of an Individualized Treatment Plan with measurable outcomes.
• Coordinates service delivery.
• Provide Crisis Prevention and Case Management services when needed.
• Monitors service delivery
