JOB SUMMARY
The Home Health Nurse Case Manager is a professional nurse who coordinates and directs the home care patient's care based on individual patient needs. The Nurse Case Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Nurse Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.
SKILLS, EXPERIENCE AND LICENSURE
Graduate of an accredited school of nursing: with an Associate Degree in nursing. Two (2) years of recent acute care experience in an institutional setting. Bachelor's degree, with one (1) year of home health care experience preferred. Current licensure in state and CPR and BLS certification. Management experience not required. Responsible for supervising home health aides and Licensed Practical Nurses. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position or transfer patients up to 50 pounds. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Valid Drivers Licensed with automobile that is in good working order and insured in accordance with the organization requirements
JOB SUMMARY
The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
SKILLS, EXPERIENCE AND LICENSURE
Graduate of an accredited practical or vocational nursing program. Current Florida licensed Practical or Vocational Nurse with at least one year experience in home health care or an acute care experience. Must have Florida Driver's license and a reliable automobile.
