The ideal candidate will assist licensed physical therapists in providing high-quality patient care in Home Care. Experience in acute care, geriatrics, and orthopedics is a plus . This role requires a strong understanding of medical terminology and physiology, as well as the ability to implement treatment plans effectively. Duties Assist physical therapists in developing and implementing individualized treatment plans for patients. Conduct therapeutic exercises and modalities under the supervision of a licensed physical therapist. Monitor patient progress and provide feedback to the supervising therapist. Educate patients and their families on treatment protocols, exercises, and home care strategies. Maintain accurate documentation of patient treatments and progress notes. Collaborate with healthcare team members to ensure comprehensive patient care. Participate in discharge planning to facilitate smooth transitions for patients leaving therapy. Skills Proficient in Homecare therapy practices and techniques. Experience or knowledge in acute care, and geriatrics is preferred. Familiarity with medical terminology relevant to physical therapy. Knowledge of physiology and its application in rehabilitation settings. Strong communication skills for effective patient interaction and education. Ability to work collaboratively within a multidisciplinary team. Commitment to providing compassionate patient care and support throughout the rehabilitation process.
