Full Job Description Job Title:
Home Health Technical Auditor / Clinical Coordinator Essential Responsibilities Audit evaluation notes, visit notes, missed visit notes, progress notes, care plans, and physician orders for completeness and accuracy. Review clinical documentation for Medicare, Medicaid, and state regulatory compliance. Track physician orders, certifications, recertifications, and unsigned documents. Coordinate communication among physicians, field nurses, therapists, patients, and caregivers. Monitor documentation deadlines and ensure timely chart completion. Assist with scheduling and coordination of nursing and therapy visits. Support quality assurance (QA) and Quality Assurance and Performance Improvement (QAPI) activities. Identify documentation deficiencies and educate clinicians on corrective actions. Review episodes for billing readiness and regulatory compliance. Participate in case conferences and interdisciplinary team meetings. Monitor quality indicators, including hospitalizations, infections, falls, and medication issues.
