In person Job Summary We are seeking an experienced, hands-on Minnesota Licensed Assisted Living Director to manage and oversee the daily operation of our assisted living facility. We need an ambitious, dedicated, and dependable leader who consistently shows up, takes ownership, follows through on responsibilities, and works every scheduled day to move the facility forward. The ideal candidate understands Minnesota Statutes, Chapter 144G; Customized Living services; assisted living operations; resident services; policies and procedures; staffing; documentation; and regulatory compliance. The LALD will ensure the facility remains survey-ready, residents receive safe and person-centered services, staff are properly hired and trained, shifts are covered, and clinical and DSP documentation is complete and consistent with each resident's needs and service plan. Key Responsibilities Leadership and Daily Operations Serve as the facility's Licensed Assisted Living Director and director of record. Manage and oversee the facility's daily operations. Provide hands-on leadership, supervision, accountability, and direction to staff. Ensure the facility operates in compliance with Minnesota Chapter 144G and other applicable laws and regulations. Maintain a safe, respectful, person-centered, and professional environment. Keep ownership informed about compliance risks, staffing concerns, resident issues, and operational needs. Establish clear responsibilities and follow up to confirm assignments are completed. Maintain a consistent and dependable on-site leadership presence. Take initiative and address concerns before they develop into larger operational or compliance problems. 144G Compliance and Survey Readiness Maintain working knowledge of Minnesota assisted living laws, rules, and regulatory updates. Ensure required Chapter 144G policies and procedures are developed, current, implemented, and followed by staff. Maintain continuous readiness for Minnesota Department of Health surveys and complaint investigations. Conduct routine audits of resident files, staff files, training records, service documentation, incident reports, and facility records. Respond to licensing findings and develop and implement plans of correction when necessary. Ensure required notices, postings, disclosures, contracts, resident rights, and emergency information are current. Maintain organized evidence demonstrating regulatory compliance. Customized Living Oversight Understand Minnesota Customized Living services and waiver-funded service requirements. Oversee the delivery and documentation of authorized Customized Living services. Monitor service authorizations, renewals, effective dates, service agreements, and other required documents. Coordinate with residents, families, case managers, counties, lead agencies, and health care professionals. Ensure services provided and billed are consistent with each resident's assessed needs, service plan, authorization, and assisted living contract. Identify upcoming authorization expirations and follow up early enough to prevent interruptions in services. Resident Services and Rights Protect each resident's health, safety, dignity, privacy, independence, choice, and well-being. Ensure services are person-centered and consistent with resident assessments, preferences, goals, and service plans. Ensure individualized plans are developed, implemented, monitored, and updated. Respond promptly to resident and family concerns, complaints, grievances, incidents, and changes in condition. Ensure staff understand and protect the Assisted Living Bill of Rights. Coordinate resident care and services with the Clinical Nurse Supervisor and other members of the resident's care team. Ensure required incidents and suspected maltreatment are documented and reported appropriately. Clinical and DSP Documentation Oversight Partner closely with the Clinical Nurse Supervisor regarding assessments, care planning, medication management, treatment management, staff competency, and delegated nursing services. Ensure clinical records and DSP documentation are completed accurately and within required timeframes. Audit DSP progress notes and service documentation for completeness, accuracy, and consistency. Confirm that staff notes reflect the resident's service plan, assessed needs, authorized services, goals, interventions, and actual services provided. Identify missing, late, copied, incomplete, or noncompliant documentation. Maintain secure and confidential resident records. Hiring, Staffing, and Shift Coverage Recruit, interview, hire, onboard, train, supervise, and evaluate qualified staff. Ensure required background studies are completed before employees have prohibited access. Maintain appropriate staffing levels based on resident needs and facility operations. Develop and monitor employee schedules. Ensure all scheduled shifts are covered and address call-offs and staffing shortages promptly. Maintain an active pool of qualified DSPs to reduce uncovered shifts and excessive overtime. Verify that employees meet all training, orientation, competency, certification, and documentation requirements. Monitor attendance, reliability, performance, professionalism, and compliance. Conduct staff meetings, coaching sessions, and performance evaluations. Take timely corrective or disciplinary action when staff do not meet expectations. Training and Employee Compliance Ensure staff complete required facility orientation before providing services. Ensure staff receive resident-specific orientation before working independently with a resident. Track annual training, dementia training, vulnerable-adult reporting, resident rights, infection control, emergency procedures, and other required topics. Maintain complete training and competency records for every employee. Ensure staff understand the facility's policies and can demonstrate how to implement them. Provide additional training when documentation audits, incidents, complaints, or performance reviews identify a need. Quality Assurance and Risk Management Establish an ongoing quality-assurance and compliance-audit system. Monitor incidents, falls, medication errors, hospitalizations, complaints, infections, staffing concerns, and documentation problems. Identify patterns and implement corrective actions to prevent problems from happening again. Ensure emergency procedures, fire drills, safety inspections, infection-control practices, and required facility checks are completed and documented. Monitor the physical facility and ensure maintenance and safety concerns are corrected promptly. Provide ownership with regular operational, staffing, resident-care, and compliance reports. Required Qualifications Current and unrestricted Minnesota Licensed Assisted Living Director license. Eligible and willing to become affiliated with the facility as its director of record. Hands-on experience managing or overseeing a Minnesota Chapter 144G assisted living facility. Strong knowledge of Chapter 144G requirements, policies, procedures, resident rights, training, documentation, and survey readiness. Knowledge of Minnesota Customized Living services and waiver-funded service requirements. Experience with DSP hiring, supervision, scheduling, shift coverage, training, and performance management. Experience reviewing resident records, clinical documentation, DSP notes, service plans, and compliance files. Demonstrated history of dependable attendance, punctuality, consistency, and professional follow-through. Ability to successfully complete all required background-study and employment requirements. Strong leadership, communication, organization, documentation, and problem-solving skills. Ability to use electronic records, scheduling, documentation, and compliance software. Ability to work on-site for 40 hours per week. Preferred Qualifications Previous experience preparing for or participating in Minnesota Department of Health surveys. Clinical and DSP documentation is accurate and completed on time. Policies and procedures are current and followed in practice. Staff are properly screened, trained, supervised, and held accountable. Resident needs are covered, and scheduled shifts do not remain unfilled. Customized Living authorizations and required documents are tracked before expiration. Residents receive safe, respectful, and person-centered services. Ownership receives consistent updates about facility operations, risks, and progress.
