We also serve as the Child and Family Connections agency for DuPage, Kane, and Kendall Counties, supporting families eligible for services through the Illinois Early Intervention system. Learn more at www.dayonepact.org. DayOne PACT is growing! We're looking for an experienced, full-time LMS Service Coordinator II to join our Life Management Services team in Lisle, IL. This role carries a full caseload across Special Needs Trust Administration, Advocacy & Private Case Management, and Standby services, plus expanded responsibility as Guardian and Power of Attorney agent for clients — including Guardianship of Person/Power of Attorney for Health Care and Guardianship of Estate/Power of Attorney for Property. The ideal candidate is an experienced human services professional with personal finance skills, including budgeting and bill payment, who is comfortable managing complex legal, financial, and administrative responsibilities with excellent interpersonal skills and judgment. This is primarily an on-site position with frequent local travel; opportunity for one remote day per week. What You'll Do
  • Provide direct services to a caseload of Trust beneficiary, Advocacy, Standby, Guardianship, and Power of Attorney clients
  • Conduct assessments and routine consultations, develop budget and care/service plans, manage disbursement requests, and monitor accounts
  • Provide information, referral, and coordination of products and services; visit individuals offsite as needed
  • Coordinate with stakeholders, including service providers, financial advisors, legal professionals, guardians, and vendors
  • Assist in preparation of annual accountings, trust tax filings, and annual court reports/accountings for guardianship cases; participate in investment performance reviews
  • Provide routine and emergency care coordination and financial/property coordination for Guardianship and Power of Attorney clients
  • Maintain accurate program documentation, including case notes, reports, and fee-for-service billing; maintain billing at 55% of hours worked.
  • Utilize effective de-escalation techniques and work to continuously improve stakeholder satisfaction
  • Provide back-up service delivery to support Service Coordinator colleagues as needed
  • Participate in on-call rotation, responding in a timely fashion to emergency and after-hours calls received via third-party answering service
  • Participate in supervisions, meetings, and trainings; pursue professional development opportunities as assigned What You Bring
  • Bachelor's degree (required) in Social Work or a related Health & Human Services field (preferred); Master's degree preferred
  • Experience working with individuals with disabilities and/or case management; prior experience with special needs trusts, guardianship, or power of attorney agency preferred
  • Certified guardian, or ability to attain guardianship certification
  • Ability to review and process complex legal, financial, and administrative documents and tasks
  • Strong interpersonal skills and a professional, empathetic approach
  • Personal finance skills, including budgeting and bill payment
  • Excellent organization, attention to detail, and time management
  • Strong computer skills, including Microsoft Office and database systems
  • A valid driver's license and willingness to travel locally (within Cook & Collar counties) What We Offer
