About this role Sobrius Health provides substance use disorder treatment in central and southwest Virginia, licensed by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) at ASAM Level 2.5 (partial hospitalization) and Level 3.5 (clinically managed high-intensity residential). The Risk and Quality Manager owns the systems that keep those programs safe, compliant, and measurably effective. This is not a paperwork role. You will sit at the center of how Sobrius learns from its own data — running root cause analyses that change practice, building performance measures clinicians actually recognize as meaningful, and keeping the organization ready enough that a licensing survey is a confirmation rather than a scramble. You will have the authority to audit any department, require corrective action, and set the deadlines for it, and you will present quality and safety findings directly to executive leadership and the governing authority. We work from a just-culture premise: incidents and near misses are information about systems, not ammunition against staff. The person in this role sets that tone. This is an in-person position and requires some travel between sites. What you will own Quality improvement 12VAC35-105-620 Maintain the written Quality Improvement Plan, with documented review and update at least annually. Set measurable goals and objectives, monitor progress against them, and report on the statewide performance measures required by DBHDS. Build and trend measures that matter clinically. Gather input from individuals receiving services and carry it into the plan as concrete improvements. Lead performance improvement projects from problem statement through sustained change. Present quality and safety reports to executive leadership and the governing authority on a defined schedule. Risk management 12VAC35-105-520 Serve as the designated risk management personnel and complete DBHDS-approved risk management training. Maintain the written risk management plan addressing personal injury, infectious disease, property damage or loss, and other sources of potential liability. Conduct the annual systemic risk assessment covering the care environment, clinical assessment and reassessment practices, staff qualifications and staffing levels, high-risk procedures, and serious incident review. Apply the uniform risk triggers and thresholds established by DBHDS, and use the Individual Risk Tracking Tool to monitor Level I, II, and III serious incidents and care concerns. Ensure annual safety inspections are completed at every Sobrius location and drive the resulting recommendations to closure. Document and retain records of serious injuries to employees, contractors, students, volunteers, and visitors, and evaluate them annually for pattern and prevention. Incident oversight and root cause analysis 12VAC35-105-160 Ensure Level II and Level III serious incidents are reported to DBHDS through CHRIS accurately and within required timeframes, and that notice reaches the individual's authorized representative and designated contacts within 24 hours of discovery. Complete or assign a root cause analysis within 30 days of discovery for each Level II and Level III incident, identifying causes within the provider's control and solutions that prevent recurrence. Maintain and apply the organization's thresholds for enhanced, team-based analysis — repeat similar incidents involving the same individual or location, and death from an unexpected acute medical event. Collect, maintain, and review all serious incidents at least quarterly; analyze trends, name the systemic issues, remediate them, and document what was done. Coordinate reporting and internal review of abuse and neglect allegations and of seclusion and restraint, consistent with the human rights regulations (12VAC35-115). Track every corrective action to verified completion — not to submission. Compliance monitoring and survey readiness 12VAC35-105-170 Run a routine audit schedule across clinical documentation and service plans, ASAM admission and continued-stay determinations, medication management, individual rights, staff credentials and supervision, training records, and confidentiality practice under HIPAA and 42 CFR Part 2. Own the corrective action plan process end to end — drafted within the required 15 business days, free of PHI and PII in CONNECT submissions, with target dates, named owners, and verification of effectiveness. Serve as the organization's point of contact for the DBHDS Office of Licensing and manage CONNECT portal submissions, renewals, and authorized contact lists. Keep policies and procedures current with DBHDS regulations, the ASAM Criteria, and payer requirements, with documented annual review. Train staff on incident reporting, individual rights, documentation standards, and risk practice — in plain language, close to the work. What we are looking forRequired Bachelor's degree in a behavioral health, healthcare, public health, or closely related field. Three or more years in behavioral health or substance use treatment, including at least one year in quality improvement, risk management, compliance, or regulatory and accreditation work. Working knowledge of the DBHDS licensing regulations (12VAC35-105), the human rights regulations (12VAC35-115), and the ASAM Criteria. Demonstrated experience with incident reporting, root cause analysis, auditing, and corrective action planning. Comfort with data — able to define a measure, pull and clean the numbers, and explain a trend clearly to both clinicians and a board. Ability to pass required background checks and maintain a valid driver's license. Travel required up to 50%. Preferred Master's degree, or Virginia clinical licensure or license-eligible status (LCSW, LPC, LSATP, LMHP-R/RP, RN). Prior experience with a DBHDS licensing survey, CARF or Joint Commission accreditation, or Virginia Medicaid ARTS requirements. Experience in a residential or partial hospitalization setting.
Pay:
$75,000.00 - $90,000.00 per year
