Nurse Practitioner - Concierge Geriatric Care (Palm Beach / Boca Raton) Full-Time | In-Home and Facilities A concierge-style geriatric practice in the Palm Beach and Boca Raton area is seeking a Nurse Practitioner to provide high-quality, individualized care for a small, dedicated patient panel. This role offers strong clinical autonomy, a balanced mix of home visits and telehealth, and the chance to help shape a growing practice.

Key Responsibilities:

Conduct comprehensive in-home and facility geriatric assessments Provide ongoing care and follow-up visits through home appointments and facility visits Collaborate with the physician team as needed while maintaining a high level of independence Support care planning, medication management, and chronic disease oversight Maintain accurate documentation and utilize

Requirements:

Active Florida Nurse Practitioner license Experience or strong interest in geriatrics Comfortable conducting in-home visits throughout Palm Beach and Boca Raton Organized, professional, and confident using technology

Start Date:

ASAP Pay:

$125,000.00 - $150,000.00 per year
