Job Description

POSITION SUMMARY
: The HUC/CNA of the Extended Care Facility is responsible to perform both clinical and administrative duties of the nursing home.
SCHEDULE
72-80 hrs per pay period; Day Shift
ESSENTIAL JOB DUTIES/REQUIREMENTS
HUC Duties:
Responsible for having or obtaining sufficient knowledge to complete requisitions, reports maintain records and perform clerical tasks. Responsible for accurately transcribing physicians orders to Pharmacy, Kardexes, Laboratory, Radiology, Rehab. Services, Home Health and others as needed. Provides a safe, clean and comfortable environment for patients. Completes admission, discharge and deceased resident paperwork. Ability to prioritize, presenting problems to be handled during the shift. Maintains at all times patient's rights. Reviews charts to maintain physician orders, nurse's notes and progress notes. Writes CBA assignments from schedule to the "brain". Develop BM list. Receives and places confidential information with courtesy, accuracy and respect at all times. Addresses patients, visitors and co-workers in a pleasant, respectful manner and displays courtesy at all times. Adheres to the guidelines set forth in the facilities Personnel Policy Manual and Policy and Procedure Manuals. Follows instructions received from licensed nursing staff (RN's & LPN's). Adheres to uniform and hygiene requirements as set forth in the Personnel Policy Manual. Reacts properly and follow correct policy and procedure in an emergency and/or disaster. Orders supplies bi-weekly. Prepares weekly bath/shower schedules. Prints, edits and files monthly resident flow records. Processes MAR's for billing on a monthly basis. Prints, sends to pharmacist for signature and files monthly drug regiment review. Prints, delivers for physician's signature and files monthly physician's orders. On a monthly basis, removes and files old RFR, MAR and DRR. Implements lab slips for monthly lab orders. Monthly report to physicians for dates on progress notes. Monthly ordering for Evergreen. Print, edit and deliver schedules to Extended Care Director of Nursing. Monthly schedule updates. Monthly CNA assignments for evenings and nights. Monthly Kardex review. Completes annual review and keeps CPR certification and TB vaccine current. Must wear Personal Protective Equipment (PPE) as situations require to maintain Infection Control standards set by the Hospital.
CNA Duties:
Successfully completes the delivery of basic care needs for 6-8 residents on days, 7-10 residents in the evenings and 10-14 residents on nights. Will perform personal hygiene and care of patients (oral care, hair care including shaving, nail care, peri care with or without attends, catheter care, AM/PM care, partial baths, complete baths, bed making, ambulation of resident, dressing residents, feeding including residents with swallowing difficulties, range of motion exercises, restraint use, hand washing), complete flow sheets and write brief statements and take vital signs. Transfers patients to and from, either assisted or unassisted, with all the following: bed, wheelchair, chair, stretcher, examination table, commode, scales and floor. Repositions patient if in a bed, wheelchair, chair, stretcher, examination table, commode, scales and floor. Provides team support to all other residents in the unit as requested during the shift. Provides direct care to partners (another CNA's) residents during shift for breaks, lunches and with difficult residents. Completes all necessary documentation of care given on assigned residents. Reports all necessary/abnormal information about the patient to the charge nurse. Follows instructions received from licensed staff (RN's, LPN's, Physicians). Participates in code situations as chest compressor or recorder in ECF, ACF or ED. Prioritizes, presenting problems to be handled during shift. Address, with supporting documentation, any questions or concerns regarding patient care with the charge nurse. Addresses patients, visitors and co-workers in a pleasant, respectful manner and displays courtesy at all times. Makes frequent visual checks (at least every 30 minutes) on assigned residents. Maintains at all times patient's rights. Preceptor new employees. Monitors the physical environment of the ECF and notifies charge nurse when necessary. Receives and places confidential information with courtesy, accuracy and respect at all times. Adheres to the guidelines set forth in the facilities Personnel Policy Manual and Policy and Procedure Manuals. Completes annual review and keeps CPR certification and TB vaccine current. Maintains required education credits per facility policy. Adheres to uniform and hygiene requirements as set forth in the Personnel Policy Manual. Reacts properly and follow correct policy and procedure in an emergency and/or disaster. Must wear Personal Protective Equipment (PPE) as situations require to maintain Infection Control standards set by the Hospital. Must submit to COVID testing as required by
CMS. NON-ESSENTIAL JOB DUTIES/REQUIREMENTS
Must be willing to assist in other areas as the need arises. Other duties as delegated.
POSITION QUALIFICATIONS
High School diploma or GED equivalency required. CNA Certificate required. BLS Required within 30 days of hire Medical terminology preferred. Basic computer skills in Micro soft word/ Excel and Outlook required. Operates copy machine and fax machine. Keyboard at 40 wpm. Prefer experience working on Meditech 6.0. Previous experience answering multi line telephone system. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. Ability to communicate in visual, audio and/or written form.
