The role of a Limited Licensed Master Social Worker (LLMSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) at Circle of Hope is an essential member of the Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) who is responsible for providing comprehensive mental health services, supporting clients with severe mental health disorders, and working closely with a multidisciplinary team to achieve optimal treatment outcomes. This role requires a blend of therapies (e.g., group therapy, individual therapy, and/or family therapy), client assessments, treatment planning, and collaboration with medical and behavioral health professionals.
Key Responsibilities:
The Limited Licensed Master Social Worker (LLMSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) duties include, but are not limited to:
Assessment and Evaluation:
The therapist will complete intake referral packets accurately and thoroughly as part of the admissions and intake process. The therapist understands that timely and comprehensive intake documentation, screening, and referrals are essential for effective treatment planning and continuity of care. The therapist understands that comprehensive screening and accurate assessment are crucial for effective treatment planning and referral to other services when necessary. The therapist will conduct biopsychosocial assessments to evaluate client needs and assist in determining appropriate treatment plans. The therapist will screen clients for mental health issues, substance use, and other psychosocial factors that may affect their treatment, referring clients to additional services, specialists, or levels of care as needed. The therapist will collaborate with the treatment team to ensure the care provided is individualized and appropriate to the client's diagnosis and level of functioning. The therapist will ensure that all assessments and evaluations are completed in accordance with facility policies and procedures, including required timelines for submission and updates.
Treatment Planning:
The therapist will initiate discharge planning at the start of treatment, identifying potential barriers to successful transition and coordinating with the treatment team to ensure continuity of care upon discharge. The therapist will provide case management services, including coordination with external providers, community resources, and family members to support the client's treatment and discharge goals. The therapist will develop, implement, and review individualized treatment plans, ensuring that these plans align with each client's therapeutic goals. The therapist will attend regular treatment plan meetings, and ensure the client is present, as well as adjust treatment strategies based on client progress to maximize client functioning and enhance departmental performance. The therapist will ensure that all treatment planning and case management documentation is completed in accordance with facility policies and procedures, including required timelines for submission and updates.
Therapeutic Interventions:
The therapist agrees to provide applicable therapies, such as group therapy, individual therapy, and/or family therapy, as well as crisis intervention as part of my clinical responsibilities. The therapist will utilize evidence-based practices (e.g., Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Acceptance and Commitment Therapy (ACT), Motivational Interviewing (MI), etc…) in the therapeutic work. Client & Family Engagement (only with a Release of Information (ROI) or a Legal Guardian): The therapist will provide supportive, respectful, and culturally responsive care to all clients and their families. The therapist will educate clients and family members about client care in an accessible and compassionate manner. The therapist will promote health literacy, engagement in treatment, and shared decision-making wherever possible.
Documentation and Recordkeeping:
The therapist is required to complete all mandatory client documentation, including but not limited to intake documentation, admission documentation, treatment planning documentation, progress note documentation, discharge documentation, and any other required clinical documentation in a timely and professional manner. The therapist is responsible for completing any FMLA (Family and Medical Leave Act) and Short-Term Disability (STD)paperwork received for PHP and IOP clients, as well as ensuring such documentation is submitted accurately and in a timely manner. The therapist will maintain accurate and up-to-date client records, including paper medical records and electronic medical records (EMR), ensuring compliance with state, federal, and facility regulations. The therapist will ensure that all entries in the paper medical record and electronic medical record (EMR) are clinically sound, reflect the services provided, and meet auditing and billing requirements. The therapist understands the importance of timely documentation and will adhere to all policies and procedures related to documentation timelines, confidentiality, and privacy standards.
Collaboration & Communication:
The therapist agrees to maintain a functional rapport with all personnel employed by Circle of Hope, as well as anyone that the organization collaborates with. If the therapist is unable to continue functional rapport with an individual, the therapist will immediately notify the Director of Operations. The therapist will work as part of a multidisciplinary team, collaborating with Psychiatrists, Nurses, Social Workers, and other healthcare professionals to deliver high-quality care to clients. The therapist understands that collaboration is not limited to direct care but includes participation in regular team meetings, client care meetings, clinical discussions, case consultation, and potential staff interviews. The therapist is required to actively participate in direct care, regular team meetings, client care meetings (including group therapy, individual therapy, and/or family therapy), client assessments, clinical discussions, and discharge planning. The therapist will contribute to treatment planning and care coordination to ensure continuity and high-quality of care across disciplines. The therapist may be asked to assist in orienting new staff to the program by sharing processes and program expectations. When needed, the therapist will provide peer consultation to support clinical decision-making and promote team development. The therapist agrees to communicate effectively with the clinical team, medical providers, and external care teams. The therapist understands that additional responsibilities may be assigned on an ongoing basis, based on organizational needs and priorities, beyond those outlined above.
Performance Improvement:
The therapist will actively participate in departmental quality assessments and quality improvement initiatives, including setting goals, collecting and evaluating relevant data, and assisting with the development and implementation of action plans to improve service delivery and client outcomes. The therapist will subject clinical case activity and documentation to supervisor and peer review as part of ongoing performance improvement efforts and to support quality assurance measures.
Compliance and Ethical Standards:
The therapist agrees to uphold the highest standards of ethical and professional conduct, including: i. Complying with all federal, state, and local laws and regulations governing Social Work practice, Counselor practice, and mental health services, ii. Following all policies and procedures of the organization, including those related to clinical documentation, client rights, and incident reporting, iii. Always maintaining professional boundaries with clients, avoiding dual relationships and conflicts of interest, iv. Practicing within the scope of a Social Worker or Counselor license and seeking guidance when uncertain about clinical or ethical decisions. I will uphold the ethical standards of the NASW or ACA and facility policies and procedures, v. Reporting any observed or suspected violations of policy, law, or professional standards to the appropriate supervisor or compliance officer, vi. Participating in mandatory trainings related to compliance, ethics, confidentiality, and safety, and vii. Upholding principles of equity, cultural sensitivity, and trauma-informed care in all interactions. The therapist understands the importance of continuing education to stay current with best practices and state regulations, as well as continuing professional growth and development.
Crisis Management:
The therapist will respond promptly and professionally to crisis situations, ensuring the safety and well-being of clients at all times. The therapist will conduct risk assessments, including evaluations for suicidal ideation, self-harm, harm to others, or significant deterioration in mental status. The therapist will assess the potential need for a higher level of care, including inpatient hospitalization or emergency psychiatric services, and coordinate referrals when clinically indicated. The therapist will develop and implement safety plans in collaboration with clients, families, and the treatment team. The therapist will document all crisis interventions and assessments thoroughly, following facility procedures and clinical standards.
Qualifications:
Education:
The therapist has a Master degree from an accredited higher education institution.
Licensing:
The therapist holds a Limited Licensed Master Social Worker (LLMSW), a Licensed Master Social Worker (LMSW), or a Licensed Professional Counselor (LPC) license in the State of Michigan. If the Social Worker holds a Limited Licensed Master Social Worker (LLMSW) license, the Social Worker agrees to work under the supervision of a Licensed Mater Social Worker (LMSW).
Experience:
The therapist has at least one year of experience in clinical social work, particularly in mental health settings, and is familiar with evidence-based therapeutic modalities.
Skills:
1) The therapist possesses the ability to understand a variety of communication forms, such as written, oral, diagram, or schedule form, 2) The therapist demonstrates the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and policy and procedures manuals, 3) The therapist is proficient in maintaining accurate documentation and working collaboratively within a team, 4) The therapist has the skills necessary for effective clinical assessments, treatment planning, and crisis intervention, 5) The therapist possesses the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists, 6) The therapist demonstrates the ability to interact with people of various backgrounds in a courteous, tactful, and poised manner, and 7) The therapist has knowledge of occupational health and safety, workers compensation, and OSHA regulations.
Physical Requirements:

Sedentary Work:
Exerting up to 10 pounds of force occasionally. Involves sitting most of the time, though may involve walking or standing for brief periods. Positions classified as sedentary may occasionally require pushing, pulling, lifting, or carrying small items (e.g., files, documents, or office supplies). Frequent use of hands and fingers for typing, writing, or handling paperwork.
Light Work:
Exerting up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently. May involve walking or standing to a significant degree, or working at a desk while performing some lifting or carrying. May involve pushing/pulling of objects or rolling carts. Requires frequent use of arms and hands for reaching, lifting, and fine motor skills.
Medium Work:
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. Requires standing and walking for extended periods. Involves frequent lifting, carrying, pushing, or pulling of materials or equipment. May include bending, kneeling, or stooping.
