Job Summary We are seeking a dedicated and compassionate Clinical Care Coordinator to join our skilled nursing facility team. The ideal candidate will serve as a vital link between patients, families, and healthcare providers, ensuring high-quality, coordinated care. This role involves overseeing patient care plans, facilitating communication among interdisciplinary teams, and supporting residents' health and well-being. The Clinical Care Coordinator will be an LPN or RN with a strong understanding of clinical procedures and patient advocacy. Responsibilities Coordinate and monitor individualized patient care plans in collaboration with healthcare teams to ensure optimal outcomes. Act as a primary point of contact for residents, families, and staff regarding care needs and updates. Assist in assessing residents' conditions, documenting clinical observations, and updating care records accurately. Facilitate communication among physicians, nursing staff, therapists, and other healthcare professionals to promote seamless care delivery. Support the implementation of treatment protocols and ensure adherence to facility policies and regulatory standards. Educate residents and their families about health conditions, treatments, and available services to promote informed decision-making. Participate in interdisciplinary team meetings to review patient progress and adjust care strategies as needed. Ensure compliance with safety protocols, infection control procedures, and quality assurance standards. Assist with training new staff members on clinical procedures and facility policies as needed. Qualifications Valid LPN or RN license in good standing. Prior experience working in a skilled nursing or long-term care facility is preferred. Strong knowledge of clinical practices, patient care coordination, and healthcare regulations. Excellent communication skills to effectively interact with residents, families, and multidisciplinary teams. Compassionate approach with a focus on resident-centered care. Ability to multitask efficiently while maintaining attention to detail in a fast-paced environment. Proficiency in electronic health records (EHR) systems is desirable.
