Job Description

We are seeking a dedicated and organized Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive comprehensive care by coordinating services and facilitating communication between patients, healthcare providers, and administrative staff. This position requires a strong understanding of medical terminology and the ability to manage multiple tasks efficiently in a fast-paced environment.
Duties:
Serve as the primary point of contact for patients, addressing their inquiries and concerns regarding their care. Coordinate appointments, referrals, and followup services to ensure seamless patient care. Maintain accurate patient records and documentation in compliance with healthcare regulations. Assist Intake Coordinator in the input/downloading of all pertinent patient information into the agency database system. Utilize medical terminology effectively to communicate with healthcare professionals and assist in administrative tasks. Provide support in managing office operations, including scheduling, billing inquiries, and insurance verification. Assist in the development of care plans tailored to individual patient needs.
Knowledge, Skills, and Experience:
Minimum Education (or substitute experience) required: Must possess, as a minimum, a high school diploma or equivalent (GED).
Minimum Experience required:
Onthe job training provided in staffing procedure. Experience in coordinating pre and posthospital care preferred. Computer Skills required both clinical and clerical programs. Must be able to type a minimum of 45 words per minute and have a working knowledge on medical terminology. Possession of strong organizational skills. Excellent verbal and written communication skills. Possess exceptional interpersonal communication skills.
