JOB DESCRIPTION

Director of Care Coordination. 13 weeks assignment with the possibility of extension for another 13 weeks. Tentative Start Date 10/05. Guaranteed hours 40 per week. Overtime is available. Must be able to work for Day shifts, Monday - Friday. Must have previous leadership exp in Care Coordination. BSN required, master's degree in nursing, Health Administration or Business Administration is preferred. Case Management certification (e.g., CCM or ACM) preferred. Minimum five (5) years of experience in an acute healthcare setting required. Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required.
