Gross Weekly Pay:

$4,800/week

Pay Rate:

$120/hour (Split as $90/hour taxable + $1,200/week stipend ) Basic Roles & Responsibilities Lead and oversee Care Coordination and Case Management operations Supervise and support interdisciplinary teams Oversee discharge planning and transitions of care Monitor patient progression and appropriate utilization of resources Drive quality and performance improvement initiatives Ensure compliance with regulatory and accreditation standards Collaborate with physicians, nurses, social workers, and other departments Develop and implement processes to improve patient outcomes and continuity of care Provide leadership, guidance, and staff development within the department Basic Requirements BSN required Master's degree in Nursing, Health Administration, or Business Administration preferred 5+ years of acute healthcare experience 5+ years of progressive leadership/administrative experience in: Case Management & Care Coordination, Utilization Management, Related healthcare functions Case Management certification such as CCM or ACM preferred Strong experience leading interdisciplinary teams and clinical/operational programs Experience with performance improvement and regulatory/accreditation readiness Advanced knowledge of case management, utilization management, discharge planning, patient progression, and transitions of care
