Job Description

Summary:
The ECM Coordinator with plays a dual role in supporting high-need members through care coordination while leveraging clinical skills to improve health outcomes. This position bridges medical and social services by coordinating enhanced care management activities, assisting providers with clinical follow-up, and ensuring patients receive comprehensive, whole-person care.
Key Responsibilities:
Conduct outreach, intake, and comprehensive needs assessments for ECM-eligible members. Develop and monitor individualized care plans in collaboration with members, families, providers, and community partners. Coordinate referrals to specialists, behavioral health providers, housing programs, and social services. Support clinical workflows by applying medical assistant knowledge (e.g., reviewing charts, assisting with health assessments, monitoring vitals if needed). Ensure continuity of care by scheduling appointments, arranging transportation, and following up on care gaps or missed visits. Serve as a liaison between patients, providers, and health plans to address barriers to care. Educate patients and families about chronic disease management, preventive care, and available resources. Maintain accurate documentation in EMR/EHR and health plan care management systems. Participate in case conferences and interdisciplinary team meetings. Provide culturally sensitive advocacy and member support.
Qualifications:
High School diploma or equivalent required Certified/Registered Medical Assistant strongly preferred. 1-2 years of experience in medical assisting plus at least 1 year in care coordination, case management, or community health. Familiarity with Medi-Cal/Medicaid, IPAs, ECM services, and local community resources. Strong communication, organizational, and problem-solving skills. Proficient in EMR/EHR systems, care management platforms, and Microsoft Office. Bilingual (English/Spanish).
