This role is responsible for providing comprehensive care management and coordination to Medi-Cal members with complex medical, behavioral health, and social needs. The ECM Care Manager will manage an assigned caseload, conduct member assessments, develop individualized care plans, coordinate services, and provide consistent outreach and follow-up to help members achieve their healthcare and personal goals. Responsibilities Provide ECM services to Medi-Cal members with complex medical, behavioral health, and social needs. Manage an assigned caseload and conduct ongoing member outreach, engagement, and follow-up. Conduct assessments and develop and update individualized care plans. Complete POFs, screenings, assessments, case notes, and required ECM documentation. Coordinate care among primary care, specialty care, behavioral health, and community providers. Track referrals, appointments, care gaps, and member progress. Help members access healthcare, social services, and community resources. Address social determinants of health, including housing, food, transportation, and benefits. Qualifications Direct Enhanced Care Management (ECM) experience strongly preferred. Experience with POFs, care plans, assessments, member outreach, and care coordination . Experience working with Medi-Cal, managed care, community health, or vulnerable populations .
EPIC/EHR
experience highly preferred. Knowledge of medical terminology r equired . Strong communication, organization, documentation, and time-management skills. Ability to manage a caseload while working independently and as part of a care team.
