Under the supervision of the Clinical Supervisor, the Care Coordinator will provide support with case management services for clients and outside individuals seeking support, including electronic documentation of all such services. The Care Coordinator must contribute to an atmosphere of care and consistency in supporting the proper care of potential and existing clients.
ESSENTIAL DUTIES
Care Coordination Coordinate and provide direct case management for clients to services such as healthcare, employment, housing, transportation, and community resources, and aiding with applications/ documentation as needed. Support with maintenance of database of community resources. Effectively communicate and coordinate referrals to and from outside agencies. Collect and electronically document all care coordination services in required databases, as per contracts. (ex. Kipu, County EHR, and SharePoint). Work closely with intake and discharge teams to provide case management and referral information. When the driver is not available, may assist with transportation. Support with referrals for applicants. Complete all necessary documentation in a timely manner, including data entry into web-based systems and/or Electronic Health Records (EHR), as required by contracts. Consult with supervisors as necessary to ensure all requirements are met as they pertain to care coordination, intake, utilization, and referrals as appropriate for program design. This position requires the employee to be available during day, afternoon, and early evening hours.
SECONDARY DUTIES
Perform other tasks as assigned by supervisor, as needed in emergencies, or supporting other staff with supervisor approval.
QUALIFICATIONS EDUCATION AND EXPERIENCE REQUIRED
Bachelor's degree or higher in related field required. One year of experience in social work or case management work required. One year of previous experience in assisting clients to access services in Santa Cruz County. Strong computer skills are required.
LICENSES/CERTIFICATIONS REQUIRED
Must become a registered recovery worker with an accredited credentialing board, CCAPP or CAADEE (may be done within 1 month of start of employment). California Class C driver license. The following are required and may be obtained after start of employment: Current CPR/First Aid certification, TB test, verification of education (transcript, copy of diploma).
KNOWLEDGE REQUIRED
Principles of casework, social work, and case management required. Computer systems and applications, including county EHR and EMR knowledge required. Community service providers and resources. Principles of eligibility determination. Bilingual (Spanish/English) preferred

Ability to present information or ideas clearly in English, both orally and in writing. This includes the ability to proofread and use correct spelling, grammar and punctuation, and the ability to write correspondence and memos. Ability to interface effectively with all levels of employees, the public and with clients; to diffuse hostility when needed. Strong proficiency in computer skills is necessary. JOB SETTING
The job is performed in an enclosed climatically controlled office. Work is completed independently and in cooperation with the Clinical Supervisor, Client Care Coordinator, Intake Coordinator, and other company personnel, the public, and clients.

PHYSICAL DEMANDS
The job is primarily sedentary in an office setting, with walking and standing as necessary to carry out the job duties. There is a frequent need to sit, talk, and hear for extended periods at computer workstation with keyboard, monitor, and mouse. Normal lifting and/or carrying is limited to approximately 15lbs, with occasional lifting up to 25 lbs. Normal or corrected talking, hearing, and seeing abilities are sufficient to perform the required tasks. Duties and responsibilities may be added, deleted or changed at any time at the discretion of management, formally or informally, either orally or in writing.
Pay:
$30.00 - $32.00 per hour
