We seek an experienced CHW Supervisor to oversee our Enhanced Care Management (CHW) Integration/ Transitions of Care Services Programs. As a leader, you will foster a high-quality, patient-centered care culture, guiding your team to achieve outstanding results. You will provide visionary leadership, support, and guidance to CHWs, ensuring alignment with team goals and deliverables. As the supervisor, you will collaborate closely with management to ensure seamless alignment with MFLC objectives, standards, and goals.Key Responsibilities:
1.Program Oversight:
a. Supervise CHW teams across Enhanced Care Management (Community- based) and CHW Integration/ Transitions of Care Services (Hospital & Community-based). b. Ensure program policies, procedures, and standards are followed. c. Ensure compliance with regulatory requirements, accreditation standards, and organizational policies. 2.Staff Management:
a. Assist with Recruiting, hiring, training, and evaluating CHW staff. b. Provide coaching, mentoring, and 1-1 collaboration with staff. c. Foster a culture of teamwork, collaboration, and open communication. 3.Clinical Operations:
a. Oversee CHW services, including care coordination, health education, and community resource navigation. b. Ensure effective communication with healthcare providers, social services, and community organizations. c. Monitor and address client concerns, conflicts, or critical incidents.Summary of Programs:
Enhanced Care Management (review of ECM services is required)ECM Program Oversight:
a.Care Management Services:
Oversee care management services, including assessments, care plans, interventions, goal expectations, and coordination, to ensure holistic care. b.Communication and Collaboration:
Ensure effective communication with CHWs, healthcare providers, Partnership Health Plan, and community resources, facilitating seamless transitions and care coordination. c.Client Progress:
Monitor and evaluate program outcomes, identifying areas for improvement and implementing best practices. CHW Integration & Transitions of Care Services (review of CHW services is required)Hospital and Community-Based CHW Services Oversight:
a.Collaboration and Communication:
Collaborate with community organizations to ensure seamless transitions are achieved and care coordination is developed, focusing on the unique short-term needs of clients and developing relationships with Hospitals and community organizations. b.Care Planning and Coordination:
Ensure effective communication and care planning addresses the unique needs of the client post-hospitalization to prevent readmissions and promote preventative care and health equity. c.Transition Planning:
Facilitate smooth transitions to community-based care, addressing social determinants of health and ensuring continuity of care through f/u PCP appointments and community resources.Responsibilities:
Team Leadership:
a. Provide leadership and oversight of the ECM and CHW programs. b. Motivate and inspire team members to perform at their best. c. Foster a positive and collaborative team environment.Performance Management:
a. Monitor team performance and provide constructive feedback. b. Caseload reviews to ensure optimal performance and that clients are seen as medically necessary. c. Manage productive and nonproductive hours to reflect client visits. d. Conduct weekly performance evaluations and reports and address performance issues promptly. e. Ensure Treatment Plans, Assessments, and CPT Codes are captured to optimize patient visits. f. Ensure authorizations are obtained/current for timely billing. g. Address any denials ASAP and notify the Manager/Director. h. Monitor Documentation to ensure encounters are closed within 24 hours. i. Monitor the CHW's timecards, meal breaks, mileage, expense reports, outreach, and performance, and provide constructive feedback. j. Maintaining the Census to reflect patients in the program and their status for treatment, including care coordination, transition, and discharge planning. k. Assigning clients to CHWs who fit the care that is needed. l. Ensure authorizations are obtained/current and TARS are submitted for timely billing. m. Address any denials ASAP and notify the Regional Manager. n. Check the fax machine daily for TAR approvals and log outcomes accordingly. o. Upload care plans and ROI to PCC for QIP reporting. p. Following up on referrals for the program. q. Ensure quality metrics are captured (PHQ-9, LOCUS, blood pressure). r. Attend presentations and health fairs to promote the program. s. Conduct weekly audit calls to ensure clients are seen as reported.Communication:
a. Serve as the main point of contact between team members and upper management. b. Ensure team members are informed about policy, procedure, and goal changes.Problem Solving:
a. Address issues and resolve conflicts within the team. b. Escalate problems to management as needed and propose solutions. c. Proactively identify potential issues and take steps to mitigate risks.Collaboration:
Foster a culture of collaboration and teamwork.Caseload:
The Supervisor will carry a small caseload, which may increase for a short duration to cover vacations, staffing shortages, and as needed.NON-ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs other duties as assigned. Ability to work independently and be self-directed and flexible. Ability to perform functions with minimal supervision. Ability to work at a high-volume level of accuracy.Job Type:
