Job Description
We are looking for a Care Coordinator that is able to work directly with our clients and caregivers. Providing a personal touch of Thoughtful Home Care dedication. Our clients are in Upland and throughout the Inland Empire and San Gabriel Valley. Bilingual preferred- Spanish Thoughtful Home Care is committed to helping and providing opportunities for individuals who have the passion, drive, and fondness for improving the lives of those in need! We are looking for a Care Coordinator who is friendly and energetic with a plan for thinking outside the box. Who can understand the needs of both our clients and caregivers. Job description Care Coordinators are a vital role at Thoughtful Home Care. The Coordinator is to provide a high level of customer service that we are known for. They will communicate frequently with office to solve problems and situations. They must be comfortable working in a dynamic environment and be able to multi-task. The Coordinator must have hands-on experience with in-home elderly care. They should foresee any potential client concerns and have solutions to solve them. The Coordinator must be friendly, emphatic, organized, and maintain meticulous notes. Initial training will be provided, but candidate must be able to think on their feet and take initiative. Must be able to work and finish tasks without direct supervision. Local travel is required to do assessments, cover shifts, and build the connection with our family's here at Thoughtful Home Care. Duties Meeting with current clients and maintaining client relationship. Initial client in-home assessments. Must be able to cover shifts, if needed. Manage field staff and give feedback. Updating clients' schedules if needed and plan of care. Recognition of caregivers and disciplinary action. Some recruiting and assistance with caregiver retention. Must be willing to travel for assessments within the Inland Empire, Riverside, and San Gabriel Valley. Mileage reimbursement. Must have a valid driver license and insurance. Current Home Care Aid certification (we can help) and TB test. CPR certified preferred. Basic understanding of labor laws. Must have a minimum of three years in-home caregiving experience. Must have excellent verbal and written communication skills. Must have computer experience and cellphone for communication.
