Full-time Pay starts at $22 to $25 per hour We are seeking an organized, dependable, and motivated Clinical Care Coordinator to support our mobile healthcare team. This position plays an important role in coordinating patients, providers, facilities, clinical operations, and administrative functions.

Responsibilities:

Coordinate patient and provider schedules Communicate with patients, providers, facilities, and internal team members Assist with new patient intake and admissions Enter and maintain accurate patient information in the EHR Coordinate supplies needed for patient visits and office operations Track provider schedules, visits, and hours Assist with onboarding new facilities Assist with onboarding new providers in coordination with the HR Director Coordinate EHR documentation and communicate with billing to help ensure timely and accurate processing Perform general administrative and office management duties Occasionally accompany or assist Nurse Practitioners during patient visits as needed Help maintain timely communication and follow-up between clinical, administrative, and billing teams

Qualifications:

Minimum 2 years wound care experience Experience with Practice Fusion and/or DrChrono preferred Proficiency with email, Outlook, Microsoft Office, and general computer systems Strong organizational and time-management skills Excellent communication and follow-up Detail-oriented and able to manage multiple priorities Reliable transportation and ability to travel to facilities or patient locations when needed This is a part-time position with the possibility of becoming full-time as the organization and patient census grow.

Pay:

$22.00 - $25.00 per hour Expected hours: 40.0 per week
