Job Description
Provides advanced discharge coordination and clerical support for the Care Management team with an emphasis on forward-facing patient and family engagement, regulatory discharge documentation, post-acute coordination, and real-time electronic tracking. The role supports timely, compliant discharge planning and serves as an operational extension of the RN Case Manager under delegated authority.
