Job Description

Overview We are seeking a compassionate and organized ECM Care Coordinator to support individuals enrolled in Enhanced Care Management (ECM). This role coordinates care, conducts member outreach and follow-up, schedules services, maintains accurate documentation, and connects members with healthcare and community resources. The ideal candidate is dependable, detail-oriented, and committed to helping individuals overcome barriers to care. Responsibilities Coordinate day-to-day Enhanced Care Management (ECM) services and support member engagement throughout the care management process. Conduct member outreach, enrollment support, follow-ups, and ongoing communication to promote participation in ECM services. Schedule and coordinate appointments, referrals, follow-up services, and care-related activities. Assist members with navigating medical, behavioral health, social service, and community-based resources. Coordinate with healthcare providers, care teams, community organizations, and other service partners to support continuity of care. Maintain accurate and timely documentation of member outreach, referrals, care coordination activities, services, and follow-up efforts. Track member eligibility, enrollment, authorizations, referrals, and service activity to ensure appropriate and timely follow-up. Support ECM claims, encounters, billing documentation, and required reporting, including identifying and resolving missing or incomplete information. Maintain accurate member information within electronic health records, care management platforms, health plan portals, and internal tracking systems. Assist with obtaining required records, documentation, and information from members, providers, and partner organizations. Connect members with resources addressing social needs, including housing, food, transportation, benefits, behavioral health, and other supportive services. Follow established ECM program requirements, documentation standards, workflows, and organizational procedures. Maintain confidentiality of member information and comply with HIPAA and applicable privacy requirements. Participate in case coordination, team meetings, audits, quality improvement activities, and other ECM operational activities as needed. Provide scheduling and administrative support related to ECM operations and perform other duties related to the position.. Skills Strong verbal and written communication skills Professional and courteous phone etiquette Strong customer service and interpersonal skills Proficiency with Microsoft Excel, spreadsheets, and data tracking Comfortable with data entry, reporting, and maintaining organized electronic records Proficiency with Microsoft Office and general computer applications Ability to learn and navigate electronic health records (EHR), healthcare portals, and care management systems Member outreach, engagement, and follow-up Appointment scheduling and referral coordination Accurate and timely documentation and recordkeeping Strong organizational and time-management skills Ability to manage multiple members, tasks, priorities, and deadlines Strong attention to detail and accuracy Problem-solving skills and ability to follow established procedures Ability to communicate professionally with members, healthcare providers, health plans, and community partners Understanding of HIPAA, confidentiality, and handling sensitive information Dependable, professional, and able to work independently and as part of a team Bilingual English/Spanish preferred
Pay:
$25.00 - $27.00 per hour Expected hours: 30.0 - 40.0 per week
