Who We Are For over 25 years, Comfort Keepers has been dedicated to elevating the human spirit through uplifting in-home care that promotes independence and enhances quality of life for seniors and adults in need. Comfort Keepers of the Western Slope is one of Western Colorado's leading home care agencies, serving clients across Mesa, Delta, Montrose, Ouray and Garfield counties. We're a locally owned, CDPHE-licensed agency in growth mode — and this role is the face of our care quality. The Role The Care Coordinator owns client relationships end to end for an assigned panel of clients: conducting in-home care consultations with prospective clients, developing Plans of Care, performing supervisory visits, and serving as the primary point of contact for clients and families. You'll spend meaningful time in the field — in client homes, meeting families, and seeing care quality firsthand. This is a relationship role with real growth ownership. You'll close new client starts from consultations, conduct quarterly care-plan reviews across your panel, and recommend additional care when client needs genuinely warrant it — always in the client's interest, never beyond it. You'll report to the Executive Director of Operations and work closely with our scheduling, nursing, intake, and HR teams.
Compensation & Benefits Pay:

What We're Looking For Required:
Valid driver's license, acceptable motor vehicle record, reliable vehicle, and willingness to travel to client homes across our Western Slope service area (mileage reimbursed) Comfort discussing services and pricing, and recommending care, in emotionally sensitive situations Strong general computer skills and comfort with scheduling/EHR software Strong communication, organization, and problem-solving skills, with the ability to manage multiple client relationships and keep documentation accurate Strongly preferred: Home care, healthcare, senior services, or case management experience CNA, QMAP, or other direct-care background — you understand the work because you've done it Experience with WellSky/ClearCare or similar home care platforms A track record of growing client relationships — retention, satisfaction, or account growth What You'll Own Care consultations & new starts: Conduct in-home consultations with prospective clients and families, assess needs, present service options professionally, and close qualified starts.
Plans of Care:
Develop complete, person-centered Plans of Care and keep them accurate, current, and survey-ready as client needs evolve. Client relationships & growth: Serve as primary relationship manager for your client panel — proactive communication, service recovery, day-30 satisfaction calls, and quarterly care-plan reviews that identify under-served needs.
Supervisory visits:
Perform required in-home supervisory visits, evaluate and document caregiver performance, and monitor client satisfaction and changes in condition — escalating clinical concerns to our Director of Nursing.
Caregiver coordination:
Communicate client preferences and care instructions to caregivers; provide matching and performance feedback to Scheduling. Documentation & quality: Maintain timely, complete documentation in WellSky; contribute to quality-improvement efforts and audits.
Backup on-call:
Provide occasional, compensated backup coverage for the after-hours phone on a shared rotation. How to Apply Submit your resume through Indeed.
