Job Description
MidState Medical Center, part of Hartford HealthCare, is seeking a Care Coordinator (LCSW) to support patient-centered care in our acute care hospital in Meriden, CT. In this role, you will use your licensed clinical social work expertise to complete psychosocial assessments, develop care plans, and coordinate services across inpatient and outpatient settings. You will collaborate closely with physicians, nurses, and community agencies to support safe discharges, reduce readmissions, and connect patients with vital resources. Working in a team-oriented, compassionate environment, you will provide counseling, crisis intervention, and education to patients and families from diverse backgrounds. You'll document in the EMR, participate in multidisciplinary rounds, and contribute to continuous improvement efforts that enhance quality and safety. This position offers the opportunity to grow within Hartford HealthCare while making a meaningful impact on the health of central Connecticut residents.
