•Care Coordinator

•to join our growing behavioral health team in Washington, DC. The Care Coordinator will work closely with clients, clinicians, community support staff, healthcare providers, and community organizations to help ensure clients receive coordinated, timely, and appropriate behavioral health and supportive services. ## Key Responsibilities Coordinate behavioral health, medical, social, and communitybased services for assigned clients. Assist clients with accessing healthcare providers, housing resources, benefits, transportation, employment services, and other community supports. Communicate with therapists, psychiatrists, nurses, case managers, substance use disorder providers, and other members of the treatment team. Help clients understand and follow their treatment and service plans. Identify barriers that may prevent clients from successfully participating in treatment. Conduct regular followup with clients regarding appointments, referrals, medications, and community resources. Assist with referrals and transitions between levels of care. Participate in treatment team meetings and care coordination meetings. Maintain accurate, timely, and complete documentation in the electronic health record. Obtain and document appropriate releases of information before coordinating with outside providers. Support continuity of care following hospitalization, emergency department visits, residential treatment, or other transitions. Monitor client progress and communicate significant concerns to the appropriate clinical staff. Maintain professional boundaries and protect client confidentiality. Follow CareNationDC policies, HIPAA requirements, DBH requirements, and applicable District of Columbia behavioral health regulations. ## Qualifications Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Sociology, Public Health, or a related behavioral health field preferred. Previous experience in behavioral health, mental health, substance use disorder treatment, case management, care coordination, or community services preferred. Knowledge of community resources in Washington, DC is strongly preferred. Strong communication and documentation skills. Ability to manage multiple clients, referrals, appointments, and followup responsibilities. Comfortable working with individuals experiencing mental health, substance use, housing, medical, and socialservice needs. Ability to work collaboratively as part of a multidisciplinary treatment team. Strong computer skills and ability to learn electronic health record systems. Must be dependable, professional, organized, and clientfocused. ## Preferred Experience Experience working with one or more of the following populations or programs is highly desirable: Mental Health Rehabilitation Services (MHRS) Substance Use Disorder (SUD) services Medicaid populations Communitybased behavioral health Case management or care coordination Hospital discharge and transitionofcare services Housing and socialservice referrals ## What We Are Looking For The ideal candidate is someone who is
