Job Description
Job Summary We are seeking a dedicated and compassionate Youth Care Coordinator to join our dynamic team. In this vital role, you will oversee the holistic care and support of youth with diverse needs, ensuring their safety, well-being, and successful integration into community and healthcare systems. Your energetic approach will drive positive outcomes by coordinating services, engaging with families, and implementing individualized care plans. Duties Conduct comprehensive intake assessments for youth, including medical history, behavioral health status, and social needs Develop and implement individualized care plans in collaboration with healthcare providers, families, and community resources Coordinate medical appointments, discharge planning, and post-acute care services to ensure seamless patient transitions Manage electronic health records (EHR) accurately while maintaining compliance with HIPAA regulations Provide mental health counseling and behavioral health support tailored to each youth's unique circumstances Facilitate communication between patients, families, medical teams, and social services to promote understanding and engagement Monitor patient progress through regular assessments, documentation, and adjustment of care strategies as needed Requirements Proven experience working with children and adolescents in healthcare or social service settings Knowledge of hospital environments, including ICU, emergency medicine, Level I & II trauma centers, and inpatient/outpatient care Familiarity with medical terminology, physiology, diagnostic evaluation, and clinical case management Experience working with individuals with substance use disorders and mental health conditions Strong skills in patient assessment, discharge planning, utilization management, and community resource coordination Proficiency in EMR/EHR systems and understanding of HIPAA compliance standards Ability to prioritize tasks efficiently in high-pressure situations while maintaining excellent patient service standards Excellent client communication skills and experience working with diverse cultural backgrounds and individuals with disabilities Join us to be part of a passionate team committed to transforming youth healthcare experiences.
