GENERAL RESPONSIBILITIES

The incumbent will provide nutritional, home maintenance and supporting services to individuals 60+ years of age. Provide companionship to a functionally impaired person and to relieve the primary care giver.

SPECIFIC RESPONSIBILITIES

Relieve primary care giver by sitting with a functionally impaired elderly person for a specific amount of time as determined by the Case Manager (no hands on care). Assist the client with light housekeeping as determined by the Case Manager. Provide nutrition and meal planning assistance. Provide nutrition and meal planning assistance. Perform other support services as assigned by the Case Manager.
